Job Summary The Disaster Case Worker is responsible for providing support and advocacy to individuals and families in need, helping them access services and resources to improve their well-being. This role involves assessing client needs, developing service plans, coordinating care, and monitoring progress.
Key Responsibilities
Conduct intake interviews and assessments to determine client needs.
Connect clients with appropriate community resources, services, and benefits.
Maintain accurate and timely documentation of client interactions and progress.
Advocate on behalf of clients with service providers and other stakeholders.
Monitor and evaluate client progress and adjust service plans as needed.
Collaborate with social workers, healthcare providers, and legal representatives.
Ensure compliance with all relevant policies, procedures, and regulations.
Qualifications
Previous experience in case management or social services is highly desirable.
Strong interpersonal and communication skills.
Knowledge of local community resources and support services.
Commitment to confidentiality and ethical standards.
Preferred Skills
Bilingual: English and local language
Crisis intervention training.
Familiarity with trauma-informed care practices.
Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s preferred).
